Pain in older people is the worst-assessed problem in all of medicine. It gets attributed to age, played down by the patient out of reserve or fear of being a nuisance, and becomes invisible as soon as communication declines.
Untreated pain then no longer shows up as a complaint but as agitation, withdrawal, refusal to get up, loss of appetite, or what those around take for a change of character. In someone with cognitive impairment, that indirect route is the rule rather than the exception.
